• A doctor or hospital must issue a Medical Certificate of Cause of Death

• If unexpected, the coroner may be involved (this can delay arrangements)

• Contact a funeral home to:

  • Collect and transport the person

  • Place them into care

Locate Important Documents

• Will

• Advance care directive

• Funeral plan or prepaid arrangements

• Identification documents

Notify Immediate Family

• Prioritise close relatives and decision-makers

• Nominate a main contact person to avoid confusion
